Bridge to home program
Magellan complete Care is proud to announce that we're implementing Bridge to Home, a program that helps members make and keep the critical seven-day follow up appointment after discharge.
Why bridge to home?
Engaging Magellan Complete Care members beyond the acute setting and into recovery is critical to reducing complications, avoiding readmission, and fostering the member’s understanding of their progress. Identifying and working out barriers to follow-up treatment –before the member leaves the hospital; is important to ensure his or her engagement. Bridge to Home helps members through the discharge transition so they can be confident in their ability to successfully manage their health at home.
How does it work?
- Magellan Complete Care members with a primary mental health or substance abuse diagnosis should have an appointment with a behavioral health provider within seven days from discharge, as close to discharge as possible.
- Please schedule both the seven-day follow-up appointment and the second appointment before you discharge the member. Contact the member’s care manager if you have difficulty scheduling his or her appointments.
- Once you’ve scheduled the appointments, complete the attached bridge to home coordination form and fax it to us at 888-656-4894 within 24 hours after the member’s discharge.
- File the Bridge to Home form in the member’s medical record.
The process is easy. As an option, you can send the form to us 24 hours a day, seven days a week. No need to call us, leave messages or wait for callbacks.
